Hi, The medics do the operation if the injury is so heavy that is does not allow waiting. It´s not a literal translation. I guess it means that normally the Truppenarzt = (army doctor close to the frontline) does not operate such severe wounds but only stabilizes the patient to get him to a surgery....

I guess that "Spucker" in this context is a short slang for "Feuerspucker". Literally this means fire-eater. As the sentence translates like "Our spitters are driving to assemble in a row" I think the author meant - maybe - some tanks or self-propelled cannons assemble ...
